Distalradiusvolarrimplate:Technicalandradiographicconsiderations
摘要: AIMTodeterminetechnicalconsiderationsandradiographicoutcomesoftheSynthesvolarrimdistalradiusplatetotreatcomplexintra-articularfractures.METHODSThisreviewhighlightstechnicalconsiderationslearntusingthisimplantsinceitwasintroducedinamajortraumaunitinNovember2011,includinganatomicalreductionandwhetherthiswasmaintainedradiographically.RESULTSTwenty-sixofthe382internallyfixeddistalradialfracturesatourunit(6.8%)weredeemedtorequirethisplateinordertoachieveoptimalfracturefixationbetweenNovember2011andMay2014.Afurtherdorsaland/orradialplatewasnecessaryin35%andvariableanglescrewswereusedin54%ofcases.Postoperatively,meanradialheight,inclination,volartiltandulnarvariancerestoredwere11.7mm,21o,4.3oand-1.2mmrespectively.Therewerenocasesofnon-unionorflexor/extensortendonrupture;onecaseoflossoffracturereduction.Overallincidenceofplateremovalwas15%withoneplateremovedforflexorandoneforextensortendonirritationCONCLUSIONTheuseofarimplateenablescontrolofchallengingfardistalfracturepatterns.However,additionalplateswererequiredtoimproveandmaintainreduction.Variableanglescrewswerenecessaryinhalfthecasestoavoidintra-articularscrewpenetration.Ifusedjudiciously,thisimplantcanachievestablefixationdespitethecomplexityofthefracturepattern. ...
