摘要： Russellbodygastritis(RBG)isanunusualtypeofchronicgastritischaracterizedbymarkedinfiltrationofMottcells,whichareplasmacellsfilledwithsphericaleosinophilicbodiesreferredtoasRussellbodies.ItwasinitiallythoughtthatHelicobacterpylori(H.pylori)infectionwasamajorcauseofRBGandthattheinfiltratingMottcellswerepolyphenotypic;however,anumberofcasesofRBGwithoutH.pyloriinfectionorwithmonoclonalMottcellshavebeenreported.Thus,diagnosticdifficultyexistsindistinguishingRBGwithmonoclonalMottcellsfrommalignantlymphoma.Here,wereportanunusualcaseofan86-year-old-JapanesemanwithH.pylori-positiveRBG.Duringtheexaminationofmelena,endoscopicevaluationconfirmeda13-mmwhitish,flatlesioninthegastricantrum.Magnificationendoscopywithnarrowbandimagingsuggestedthatthelesionwasmostlikelyapoorlydifferentiatedadenocarcinoma.BiopsyfindingswereconsistentwithchronicgastritiswithmanyMottcellswithintranuclearinclusionsreferredtoasDutcherbodies.EndoscopicsubmucosaldissectionconfirmedthediagnosisofRBGwithkappa-restrictedmonoclonalMottcells.MalignantlymphomawasunlikelygiventhepaucityofcytologicalatypiaandKi-67immunoreactivityofmonoclonalMottcells.ThisisthefirstreportedcaseofRBGwithendoscopicdiagnosisofmalignanttumorandthepresenceofDutcherbodies. ... （共8页）
