Transitionclinicattendanceisassociatedwithimprovedbeliefsandattitudestowardmedicineinpatientswithinflammatoryboweldisease

摘要: AIMToevaluatedthedifferencesinknowledge,adherence,attitudes,andbeliefsaboutmedicineinadolescentswithinflammatoryboweldisease(IBD)attendingtransitionclinics.METHODSWeprospectivelyenrolledpatientsfromJuly2012toJune2013.Alladolescentswhoattendedatertiarycentre-baseddedicatedIBDtransitionclinicwereinvitedtoparticipate.Adolescentcontrolswererecruitedfromuniversity-affiliatedgastroenterologyoffices.Participantscompletedquestionnairesabouttheirdiseaseandreportedadherencetoprescribedtherapy.BeliefsinMedicineQuestionnairewasusedtoevaluatepatientsattitudesandbeliefs.Beliefsofmedicationoveruse,harm,necessityandconcernswereratedonaLikertscale.Basedonnecessityandconcernratings,attitudeswerethencharacterizedasaccepting,ambivalent,skepticalandindifferent.RESULTSOnehundredandtwelveadolescentswereincludedand59attendedtransitionclinics.Self-reportedadherencerateswerepoor,withonly67.4%and56.8%ofpatientsonanyIBDmedicationwereadherentinthetransitionandcontrolgroups,respectively.Adolescentsinthetransitioncohortheldsignificantlystrongerbeliefsthatmedicationswerenecessary(P=0.0035).Approximately20%ofadolescentsinbothcohortshadacceptingattitudestowardtheirprescribedmedicine.However,comparedtothecontrolgroup,adolescentsinthetransitioncohortwerelessskepticalof(6.8%vs20.8%)andmoreambivalent(61%vs34%)(OR=0.15;95%CI:0.03-0.75;P=0.02)totreatment.CONCLUSIONAttendanceatdedicatedtransitionclinicswasassociatedwithdifferencesinattitudesinadolescentswithIBD. ...
