Centralpancreatectomyforpancreaticschwannoma:Acasereportandliteraturereview

摘要： SchwannomasaremesenchymaltumorsoriginatingfromSchwanncellsinperipheralnervesheaths.Althoughthetumorcanbelocatedinanypartofthehumanbody,themostcommonlocationsarethehead,neck,trunkandextremities.Pancreaticschwannomasarerare.Toourknowledge,only64casesofpancreaticschwannomahavebeenreportedintheEnglishliteratureoverthepast40years.Inthispaper,wepresentapancreaticschwannomaina59-year-oldfemale.Ultrasound,computedtomographyandmagneticresonanceimagingrevealedthetumorlocatedinthepancreaticbody;however,accuratediagnosiswashardtoobtainpreoperativelyandapancreaticcystadenomawaspreliminarilyconsidered.Duringlaparotomy,themasswasfoundinthebodyofthepancreas.Anenlargedgallbladderwithmultiplestoneswasalsoobserved.Weperformedcentralpancreatectomy,end-to-sidepancreaticojejunostomyandcholecystectomy.Notably,centralpancreatectomyhasbeenreportedinonlyonecasepriortothisreport.Thegrossspecimenshowedamasswithathincapsule,1.6cm×1.1cm×1.1cminsize.Microscopicexaminationshowedthatthetumorwasmainlycomposedofspindle-shapedcellswithpalisadingarrangementandnoatypia,whichisconsistentwithabenigntumor.Bothhypercellularandhypocellularareaswerevisible.ImmunohistochemicalstainingrevealedstronglypositiveresultsforproteinS-100.Finally,thetumorwasdiagnosedasaschwannomaofthepancreaticbody.Postoperatively,thepatientrecoveredwellandleftthehospital6dlater.Duringthe53-mofollow-upperiod,thepatientremainedwellandfreeofcomplications. ... （共8页）
