摘要: Anestimated285millionadults(aged20-79years)worldwidewerediagnosedtohavediabetesmellitus(DM)in2010,andthisnumberisprojectedtogrowto439millionadultsbytheyear2030.Orthopaedicsurgeons,regardlessoftheirsubspecialtyinterest,willencounterpatientswithDMduringtheircareersincethisepidemicinvolvesbothdevelopedandemergingcountries.Diabetesresultsincomplicationsaffectingmultipleorgansystems,potentiallyresultinginadverseoutcomesafterorthopaedicsurgery.ThepurposeofthisreviewistodiscussthepathophysiologyofDManditspotentialforimpactingorthopaedicsurgerypatients.Diabetesadverselyaffectstheoutcomeofallorthopaedicsurgerysubspecialtiesincludingfootandankle,upperextremity,adultreconstructive,pediatrics,spinesurgeryandsportsmedicine.Poorlycontrolleddiabetesnegativelyimpactsbone,softtissue,ligamentandtendonhealing.Itisthecomplicationsofdiabetessuchasneuropathy,peripheralarterydisease,andendstagerenaldiseasewhichcontributestoadverseoutcomes.Wellcontrolleddiabeticpatientswithoutcomorbiditieshavesimilaroutcomestopatientswithoutdiabetes.Orthopaedicsurgeonsshouldutilizeconsultantswhowillassistininpatientglycemicmanagementaswellasoptimizinglongtermglycemiccontrol. ...
