摘要： Traditionalrightventricular(RV)apicalpacinghasbeenassociatedwithheartfailure,atrialfibrillationandincreasedmortality.ToavoidthenegativeconsequencesofRVapicalpacingdifferentstrategieshavebeendeveloped,amongtheseaseriesofpacingalgorithmsdesignedtominimizeRVpacing.ThesefunctionsareparticularlyusefulwhenthereisnottheneedforcontinuousRVpacing:intermittentatrioventricularblocksand,mainly,sinusnodedisease.However,inordertoavoidRVpacing,theoperationalfeaturesofthesealgorithmsmayleadtoadverse(oftenunder-appreciated)consequencesinsomepatients.Wedescribeacaseofapatientwithsinusnodedisease,inwhomrightatrialonlypacinginvolvedlongatrio-ventriculardelaytoallowintrinsicventricularconduction,whichledtosymptomatichypotensionthatcouldbeovercomeonlybyforcingalsorightventricularapicalpacing.Wesubsequentlydiscussthiscaseinthecontextofcurrentavailableliterature. ... （共4页）
